If you are a new client we ask that you kindly arrive 10 minutes prior to your scheduled time to complete a brief health intake form.

If you need to cancel or reschedule, please do so at least 24 hours in advance to avoid a late cancellation fee.

Please note that a valid credit card is required to secure your online reservation, though you will not be charged until the time of your service.

If you are pregnant, have recently undergone surgery, or have specific medical conditions, please list them in the booking notes so we can ensure the right treatment methods are used.

Privacy & Data Protection: I consent to the collection, storage, and handling of my sensitive health information. My data will be kept strictly confidential in accordance with the Australian Privacy Principles (APPs) under the Privacy Act 1988 (Cth). It will only be used for my clinical care and will not be shared without my explicit written consent, unless required by law.
